COPDEND has developed a standard reference pro-forma that can be used by employers and Area Teams/Health Boards when a new dental foundation trainee commences work. This has been developed in consultaion with the Dental Schools Council and NHS England.

Please note that employers are required to seek references, as are NHS England Area Teams as part of the NHS Performers List process.
